Information for New and Existing Patients

Services Heart Mountain Volunteer Medical Clinic provides:

  • Outpatient primary medical care
  • Chronic disease management
  • Wellness & health education (preventive care)
  • Basic diagnostic studies (labs and x-rays)
  • Prescription medications at little or no cost
  • Patient referral/resource network to specialized services (medical and non-medical)


Our services are primarily for adults. We do offer limited services for children, when an appropriate medical professional is available.

Patient Eligibility Requirements:

  • Reside or work in the Big Horn Basin area of Wyoming
  • Do not have health insurance
  • Gross income at or below 200% of the Federal Poverty Level (FPL; see below for more information)
  • Do not qualify for any other healthcare program such as Medicare, Medicaid, etc.
