Reminders for Current Patients

Please remember:
  • Call us at least 24 hours in advance if you cannot make it to your appointment.
    • We often have patients seeking an appointment. If you do not show up for an appointment, and do not give us 24 hours notice, another patient may have to go without care.
    • Repeated failure to let us know in advance, when you are not able to come to your appointment, may lead to a six month, one year or permanent denial of our services to you. Please don't let this happen!
  • If your income, housing and/or employment situation has changed since your last appointment with us, please bring evidence of your new situation.
  • We need to recalculate patient eligibility each time your situation changes, or at least once every 12 months.
  • Please schedule follow-up appointments before leaving the Clinic.
  • Please schedule appointments for prescription refills well in advance of the day you will run out of medication. Our schedule is often full a number of weeks in advance.
  • We look forward to helping you keep and improve your health!
